I AGREE WITH THE TERMS AND CONDITIONS STATED BELOW. I CERTIFY THAT I AM BOTH 18 YEARS OF AGE AND THE ABOVE NAMED REGISTRANT. IF THE BELOW DESCRIBED PARTICIPANT IS UNDER 18, I CERTIFY THAT I AM HIS/HER PARENT OR LEGAL GUARDIAN AND AM AGREEING TO THE TERMS AND CONDITIONS OF THIS RELEASE AND PERMISSION AGREEMENT ON PARTICIPANT'S BEHALF.

In consideration of my acceptance of this entry, I hereby agree for myself, my heirs, executors, representatives and administrators to waive and release any and all rights and claims for damages I may have against the MDS Foundation, Inc. (“MDSF”) associated with the MDSF Move for MDS Walks (the "Event"), its administrators, representatives, successors and assigns and will hold them harmless for any and all injuries I may suffer arising out of or in connection with the Event. Also, none of the above are responsible for the loss or damage of personal items or any other form of liability or aggravation in connection with the Event. I have been warned I must be in good health to participate in the Event and I am voluntarily participating in the Event. I also hereby consent to permit emergency treatment in the event of injury or illness.

I agree that it is my responsibility to pay for any medical treatment. I hereby assume all risks associated with or arising from any such medical treatment and agree to hold MDSF, coordinating groups and individuals associated with the Event, their heirs, executors, representatives, successors and assigns harmless from and against any and all claims I may have arising from or relating to such medical treatment.

I hereby grant permission to MDSF to photograph, film, audio or videotape, digitally image or otherwise record, reproduce or take any likeness of me in any medium, individually or with others, or otherwise in connection with this event ("collectively Photographs”), and to record, reproduce or transcribe in any medium my voice, comments, remarks, responses and other written or oral presentations or sound effects made by me ("collectively Recordings”), and to use, publish, exhibit, display, copyright, license, sublicense, transfer, edit, disseminate, or in any other way use or exploit ("collectively Publication”) in any language, form or media, whether now or hereafter existing, without prior approval, the Photographs and Recordings, and any parts or excerpts thereof, with or without my name associated therewith, and with or without the names or likenesses of any fictitious or other real persons, for any purpose whatsoever, including, without limitation, in connection with this event or otherwise, or the exhibition, theatrically, on film, television, or otherwise, of any production in which same may be used or incorporated, and the advertising, exploiting, or publicizing of any such production, and for any educational, commercial, fundraising, trade and pictorial art, or any other purposes. I hereby release and agree to hold MDSF harmless from any and all claims or liability for damages for libel, slander, invasion of privacy or other claims based on the Publication of any Photographs or Recordings.
